If she had work done then maybe, but not in this instance.

I don’t think she is exempt, but in this case her appearance is caused by a medical condition. Sometimes going in on someone because of what they look like can backfire.
She’s got Huntington's disease - NHS which has nothing to do with the plastic surgery she’s had.
She is also in bed with murderers, so deserves all the shit she gets.

A friend of mine’s wife is in the last stages of Huntingdon’s, tragically. It’s a hideous, cruel, devastating disease but, AFAIK, it doesn’t affect one’s appearance other than various paralysis.
