What does AT & CT mean?

Association Trained and Club Trained.

Association Trained effectively means that they spent three years (or three full seasons) with a club in the national association up to and including the season in which they turned 21. If they spent those three years with the club they are representing they count as Club Trained.

It applies irrespectively of nationality so homegrown can sometimes be misinterpreted. For example, Fabregas would count as Homegrown but Eric Dier doesn’t.

You must be looking at last season’s handbook. The cut-off this season is 1 January 1999 (Rule44. 11)
A player may be registered on List B if he is born on or after 1 January 1999 and has been eligible to play for the club concerned for any uninterrupted period of two years since his 15th birthday by the time he is registered with UEFA. Players aged 16 may be registered on List B if they have been registered with the participating club for the previous two years without interruption.
