I think this is a case of poor communication by Starmer leading to people (wilfully or accidentally) misunderstand what is actually being proposed, as the interview isnā€™t the place to go into detail.

I believe during the interview he gave two examples where this might be applied Which implies its not for everything) - back pain and internal bleeding. The former is already open to self referral where I live so I assume is also the case in some other areas, and if I thought I had internal bleeding I would probably go to A&E rather than my GP anyway. Presumably the key point is that if you know you have internal bleeding either the symptoms are obvious or there has been prior engagement with medical services for something which is probably quite urgent so why tie up GPs with that aspect of the referral?

I didnā€™t hear the interview but that struck me as a rather odd example. I saw a comment from a doctor on Twitter that he was probably referring to blood in the stool. Iā€™m not sure what happens in England but NHS Scotland used to screen for this in all over-50s.

There are a number of possible causes for that (it was colorectal cancer that we screened for) but it is only a medical emergency if it is actively bleeding or looks like treacle. Iā€™m assuming that Stanmer was talking about getting a colonoscopy or similar but that still needs some sort of triage.

Having said that, the one thing that annoys me with the German health service is how badly joined up it is. Self-referral is all well and good when you know where you meant to be referring to.

It must be the same across the whole NHS. Thereā€™s a statistical model that they use for these. I know I had a colonoscopy under 50 as I had been suffering from anaemia.

I think for a set number of specialists like (Dietitian, Physiotherapist, Pediatrician, Dermatologists, Phycologists) you should be able to self-refer. You could prescreen. with a nurse, you could place a charge (eg 50 pound) so it was not abused.

Perhaps the system could utilise NHS helpline to allow referrals that way.

Many a time going to the GP, is like calling up an internet service provider. Despite knowing what you need, they still make you go through a checklist.

1/ Turn off and on your router
2/ Try a different device
3/ Try a different cable
4/ Plug into master telephone socket

Is there a reason why GPs in particular or are they just looking for anyone to chat to?

How about receiving a ā€˜did not attend letterā€™ for a doctors appointmentā€¦that you have never made, were not in the same county and then told you have made another appointmentā€¦oh no I havenā€™tā€¦asking the health centre to sort the mess out, leaving 3 phone messages on 3 separate days, and still a month laterā€¦no one has contacted meā€¦I think they owe me money, for pointing out there is a glich in their systemā€¦

ā€¦and you end up getting blacklisted for a no-show

Because I contacted and pointed out that I didnā€™t make the appointment, it was someone elseā€¦and just by chance a doctor took my message ā€¦the ā€˜black markā€™ was taken off my recordā€¦but no apologyā€¦

Another call tomorrow just to confirm the black mark has actually been removed might be a sensible option alsoā€¦ !

Iā€™ve already called 3 times since the 5th of Januaryā€¦and still no one has repliedā€¦theyā€™re praps thinking Iā€™ll forget about itā€¦oh they are wrong!!!

He would make an appointment every week to tell the doctor how he was feeling as the doctor needed to know. Sent the bloke a bottle of wine when FIL passed.

Iā€™m still not quite sure what to make of this. Was this because he had some chronic condition or was he just looking for the company?
